Investigation Lessons Learned: Poor Records Can Weaken Good Decisions

A common lesson from workplace investigations is that poor records can weaken otherwise sound decisions.

Many organisations make reasonable decisions at the time, but later struggle to explain or defend those decisions because the records are incomplete, unclear or missing.

In an investigation, records matter.

They help establish what occurred, when it occurred, who was involved and what action was taken. They also help show whether a matter was handled fairly and consistently.

Useful records may include:

Emails and correspondence
File notes
Meeting notes
Text messages
Rosters and timesheets
Performance records
Policies and procedures
Complaint records
Training records
Incident reports
Photographs or CCTV material

Poor documentation can create significant problems.

A manager may say that a conversation occurred, but there may be no record of it. A staff member may say they raised a concern, but there may be no written complaint. A decision may have been made, but the reasons may not have been recorded.

This can leave an organisation exposed.

When records are missing, investigators may need to rely more heavily on witness accounts. That can be difficult where memories differ, time has passed, or people have different interpretations of the same event.

Good records do not need to be complicated. A short, accurate file note made at the time can be extremely valuable.

The key is to record important matters clearly and fairly.

A good record should identify the date, the people involved, the issue discussed, any response given, and any agreed next steps. It should be factual and avoid unnecessary opinion or emotional language.

The lesson is simple: if a decision may matter later, record it properly at the time.
